Fluorish AI is the technology/software platform acting on Smile Makeover's behalf to deliver these messages. Fluorish AI is not the sending identity the patient sees — every message identifies Smile Makeover by name, per the exact live wording below.
2. Website opt-in (publicly verifiable, live)
Visit Smile Makeover's public booking page — no login or account required:

- Select an appointment type, date, doctor, and time.
- Click Book Now — a phone-verification step opens.
- Enter a phone number and choose "Text me a code" — this reveals two independent, unchecked consent checkboxes shown together below.
CHECKBOX 1 — PATIENT-CARE / SERVICE SMS (unchecked by default)
I agree to receive text messages from Smile Makeover related to my appointments and dental care, including scheduling, confirmations, reminders, rescheduling/cancellations, patient-service conversations, consultation follow-up, requested forms/photos/information, treatment or case status updates, insurance/billing administrative messages, secure payment links, post-visit follow-up and recall messages. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of receiving dental care or booking an appointment.
CHECKBOX 2 — MARKETING / PROMOTIONAL SMS (separate, optional, unchecked by default)
I agree to receive occasional promotional and marketing text messages from Smile Makeover. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of receiving dental care, booking an appointment or purchasing any service.

Marketing consent is never sufficient by itself, and never required: the Send-code button is gated only by the patient-care checkbox. A patient who declines SMS entirely can choose "Email me a code" instead — zero SMS consent is required or shown on that path, and the primary action (identity verification, booking) completes normally.
This behavior is reproducible live at the booking URL above at any time — it is not a static claim.
3. One-time verbal request (inbound AI receptionist calls)
When a patient calls Smile Makeover and asks about booking, Smile Makeover's AI receptionist may ask, verbatim:
"Would you like Smile Makeover to text you one link to your phone for booking and uploading photos? This is a one-time text. Message and data rates may apply. Reply STOP to opt out or HELP for help."
- Only an explicit affirmative answer results in a text being sent.
- This authorization applies only to that one requested link — it does not enroll the caller in recurring patient-care SMS.
- It does not enroll the caller in marketing/promotional SMS under any circumstance.
4. Other web presences
This SMS program uses only the opt-in paths documented above. Additional web properties (for example, independently operated marketing landing pages or funnels) that Fluorish AI does not build, host, or control are not part of this SMS program and are not relied upon as opt-in evidence here, regardless of any SMS-related language they may separately display.
